DATA PROTECTION NOTICE

 

The following Data Protection Notice ("Notice") outlines the guidelines in which Theratech.AI Pte. Ltd. ("we", "us", or "our") may gather, employ, reveal, or otherwise handle the personal information of our customers in accordance with the Personal Data Protection Act ("PDPA"). This notice applies to personal information that is under our control or in our possession, including personal information under the control of entities we have engaged with to gather, utilize, reveal, or process personal data for our objectives.

 

PERSONAL DATA

 

  1. As used in this notice:
    "Customer" means an individual who (a) has contacted us through any means to find out more about any goods or services we provide or (b) may, or has, entered into a contract with us for the supply of any goods or services by us; and "personal data" means data, whether true or not, about a customer who can be identified: (a) from that data; or (b) from that data and other information to which we have or are likely to have access.
  2. The personal data we collect from you may vary depending on the type of interaction you have with us. This could include details such as your name and identification information (e.g., NRIC number), contact information (e.g., address, email, phone number), nationality, gender, date of birth, marital status, photographs and audio-visual information, employment details, and financial information such as credit card, debit card or bank account numbers.
  3. Other terms used in this notice shall have the meanings given to them in the PDPA (where the context so permits).
     

COLLECTION, USE, AND DISCLOSURE OF PERSONAL DATA

 

  1. Typically, we do not collect personal data unless you provide it to us directly or through an authorized representative who has been given permission by you to disclose the information. In such cases, we will inform you (or your representative) of the reasons for collecting the data and seek written consent before doing so. However, there are situations where collection and use of personal data without consent is permitted by the PDPA or other applicable laws. If we need to collect additional personal data or use it for a purpose that was not previously communicated to you, we will seek your consent (unless permitted or authorized by law).
  2. We may collect and use your personal data for any or all of the following purposes:
    1. performing obligations in the course of or in connection with our provision of the goods and/or services requested by you;
    2. verifying your identity;
    3. responding to, handling, and processing queries, requests, applications, complaints, and feedback from you
    4. managing your relationship with us;
    5. processing payment or credit transactions;
    6. sending you marketing information about our goods or services, including notifying you of our marketing events, initiatives and promotions, membership and rewards schemes, and other promotions;
    7. complying with any applicable laws, regulations, codes of practice, guidelines, or rules, or to assist in law enforcement and investigations conducted by any governmental and/or regulatory authority;
    8. any other purposes for which you have provided the information;
    9. transmitting to any unaffiliated third parties, including our third-party service providers and agents, and relevant governmental and/or regulatory authorities, whether in Singapore or abroad, for the purposes mentioned above; and
    10. any other incidental business purposes related to or in connection with the above.
       
  3. We may disclose your personal data:
    1. where such disclosure is required for performing obligations in the course of or in connection with our provision of the goods or services requested by you; or
    2. to third-party service providers, agents, and other organizations we have engaged to perform any of the functions listed in clause 5 above for us.
  4. The purposes outlined in the preceding clauses may continue to be relevant even if your relationship with us has been terminated or modified in some way. This may apply for a reasonable period following the termination or modification of the relationship, including a period required to enforce any contractual rights we may have with you.
     

PROTECTION OF PERSONAL DATA

 

  1. We have implemented administrative, physical, and technical measures to protect your personal data from unauthorized access, collection, use, disclosure, copying, modification, disposal, or similar risks during storage and transmission. We ensure that personal data is shared with our authorized third-party service providers and agents on a need-to-know basis, both internally and externally.
  2. Please note that no method of transmitting data over the Internet or storing it electronically can be completely secure. Despite our efforts, we cannot guarantee the security of your information. However, we are committed to ensuring the security of your data and regularly review and enhance our information security measures.

 

ACCURACY OF PERSONAL DATA

 

  1. We primarily rely on the personal data you (or your authorized representative) provide us. To maintain the accuracy, completeness, and currency of your personal data, please inform our Data Protection Officer in writing or via email using the contact information provided below if there are any changes to your personal information.
     

RETENTION OF PERSONAL DATA

 

  1. We may keep your personal data for as long as it is necessary to achieve the purpose for which it was collected or as required or permitted by relevant laws. Furthermore, we comply with the AHCP and MOH regulations concerning the retention of medical records for the period specified by them.
     

TRANSFERS OF PERSONAL DATA OUTSIDE OF SINGAPORE

 

  1. We typically do not transfer your personal data to countries outside of Singapore. However, suppose such a transfer is necessary. In that case, we will obtain your consent and ensure that your personal data is protected with a standard comparable to that provided under the PDPA.

EFFECT OF NOTICE AND CHANGES TO NOTICE

 

  1. This notice should be read in conjunction with any other relevant notices, contractual clauses, and consent clauses that pertain to the collection, use, and disclosure of your personal data by us.
  2. We reserve the right to modify this notice without prior notice. You may check if any revisions have been made by referring to the date of the last update. Your continued use of our services will signify your acknowledgment and acceptance of any changes made.
